Similar to other adventure games, gameplay involves finding various objects in the game world in order to combine and use them to solve puzzles, and talking to non-player characters to learn more about the world and how to progress. Set in 1944, the game follows 10-year-old girl Fran as she struggles with mental illness after witnessing the murder of her parents. She was then found alone in the woods and committed to the Oswald Mental Asylum, separating Fran from her black cat and only friend, Mr. Midnight. Under the care of psychiatrist Dr. Marcel Deern, Fran received medication. The pills induce vivid hallucinations of hallucinatory parallel universes, filling Oswald Asylum with blood, cryptic messages, horrific torture, psychiatric and neurological studies of human subjects, and otherworldly shadow creatures. Desperate to escape imprisonment, find her cat and return home, Fran escapes and flees into the nocturnal forest, where she encounters ghostly nature spirits. Fran then finds himself trapped with Mr. Midnight in a haunted house belonging to conjoined twins Clara and Mia Bhulmet. Fran eventually escapes and tries to cross a bridge of tree roots, but the bridge wears down and collapses. She wakes up to find that she has been transformed into a tree to a brighter world called Ithersta, a land where vegetables, insects, pine cones and roots live in harmony. There, she meets Palontras who helps her return home. After leaving Ithersta, Fran encounters Itward, a giant skeletal creature. She discovers Itward is helping her find Mr. Midnight. Itward tries to bring Fran home with his flying machine.

However, the craft crashed and Flambow and Itward were separated. Fran wakes up near her house and she is taken away by Dr. Dean. After visiting the grave of Fran's parents, Remo, the demonic creature that has been hunting Fran throughout the game, captures them. She is trapped in a hellish dimension ruled by Remor's mother, Mabuka. It is revealed that Fran's Aunt Grace and Dr. Oswald experimented on Grace and Fran's mother, they had Remo possess Fran in order to murder her parents, thinking that Fran's suffering was his experiment The essential. After Oswald shot her, Itward and Palontras came to Fran's rescue and killed Grace and Oswald. The revived Fran, Mr. Midnight, Itward and Palontras fly away, and Fran is determined to be happy.
